How to access the request form
Access to the Embryo Transfer service request form is mainly reserved for users who use SPID (Italian Public Digital Identity System).
Login with SPID
If you do not have a SPID you must first register
Fill out the form to register
after registering, you can log in with the username and password you created

How to submit a request for Embryo Transfer
Log in and follow the instructions to fill out the Embryo Transfer request form.
Once completed, the form will be sent as a PDF document to the email address you indicated when entering the data.
The PDF file received must be electronically signed and sent to the email address: embryo@vet.unipi.it.
If you do not have an electronic signature, you must print the PDF, sign it manually, scan it and send it to the email address embryo@vet.unipi.it, attaching a copy of a valid identity document and the tax code of the owner or the owner of the company that owns the mare for which the booking is made.
Forms without the indicated documents will not be considered valid and the request will not be processed.
